چکیده

Muscular dystrophies are a heterogeneous group of genetically inherited disorders whose most prominent clinical feature is progressive degeneration of skeletal muscle. In several forms of the disease, the function of cardiac muscle is likewise affected. The primary defect in this group of diseases is caused by mutations in myocyte proteins important to cellular structure and/or performance. That being stated, a growing body of evidence suggests that the development of autonomic dysfunction may secondarily contribute to the generation of skeletal and cardio-myopathy in muscular dystrophy. Indeed, abnormalities in the regulation of both sympathetic and parasympathetic nerve activity have been reported in a number of muscular dystrophy variants. However, the mechanisms mediating this autonomic dysfunction remain relatively unknown. An autonomic reflex originating in skeletal muscle, the exercise pressor reflex, is known to contribute significantly to the control of sympathetic and parasympathetic activity when stimulated. Given the skeletal myopathy that develops with muscular dystrophy, it is logical to suggest that the function of this reflex might also be abnormal with the pathogenesis of disease. As such, it may contribute to or exacerbate the autonomic dysfunction that manifests. This possibility along with a basic description of exercise pressor reflex function in health and disease are reviewed. A better understanding of the mechanisms that possibly underlie autonomic dysfunction in muscular dystrophy may not only facilitate further research but could also lead to the identification of new therapeutic targets for the treatment of muscular dystrophy.

Autonomic regulation in muscular dystrophy

Muscular dystrophies are a heterogeneous group of genetic muscle diseases characterized by progressive muscle weakness and atrophy. Cardiomyopathy and congestive heart failure or conduction system abnormalities that cause arrhythmias, and sudden cardiac death have been observed in muscular dystrophies. Despite the extensive research and ongoing clinical trials in muscular dystrophy to improve s...

The link between stress disorders and autonomic dysfunction in muscular dystrophy

Muscular dystrophy is a progressive disease of muscle weakness, muscle atrophy and cardiac dysfunction. Patients afflicted with muscular dystrophy exhibit autonomic dysfunction along with cognitive impairment, severe depression, sadness, and anxiety. Although the psychological aspects of cardiovascular disorders and stress disorders are well known, the physiological mechanism underlying this re...

Spinal and Bulbar Muscular Atrophy - Clinical Features and Pathogenesis

Clinical features SBMA, or Kennedy’s disease, is an inherited lower motor neuron disease characterised by adult-onset muscle atrophy, weakness, contraction, fasciculations, and bulbar involvement. The onset of weakness is usually between 30 and 50 years, but often preceded by nonspecific symptoms such as tremor, muscle cramps and fatigue. Muscle atrophy and weakness are predominant in the tongu...

Evaluation of heart involvement in calpainopathy (LGMD2A) using cardiovascular magnetic resonance.

INTRODUCTION Cardiac dysfunction occurs in several forms of limb girdle muscular dystrophy (LGMD). The aim of this study was to investigate cardiac involvement in calpainopathy (LGMD2A). METHODS Cardiovascular evaluation was performed in 10 patients with genetically verified LGMD2A by echocardiography, 3 Tesla - cardiovascular magnetic resonance, 24-h electrocardiography recordings with heart...

Autonomic regulation in muscular dystrophy

Autonomic Nervous System (ANS) function has been assessed in several muscular dystrophies. In the last two decades it has been recognized a significant relationship between ANS and cardiac mortality (Palladino et al., 2008). Cardiovascular ANS is frequently studied by using cardiovascular autonomic reflex tests and spectral analysis of heart rate in time and frequency domain (Ewing and Clarke, ...
